Its not about good or bad work. The coaches need to satisfy a lot of constraints. Like distance between coach roof and OHE wires, clearance at bridges and tunnels, clearances from platforms and other track-side structures, curve radius and many other constraints.
While IR does provide the prescribed clearances as much as possible, there are always some lapses in the construction (due to a wide variety of reasons). The LHB DD coaches themselves infringe some of these constraints on some routes and were only allowed after rigorous safety trials.
